Supported Housing is viewed as the final transition phase in our agency's continuum of care. The Supported Housing Program serves homeless individuals who have a co-occurring substance abuse history and mental illness.
The agency's Supported Housing Program is funded through a HUD grant and is designed to help reduce chronic homelessness by assisting the individual in gaining permanent housing through participation in vocational, educational services that will enhance their ability to become self-sufficient. The program is staffed by a Housing and Vocational Specialist that help provide linkage and referral to appropriate services i.e. Mental Health, Housing, Vocational Training and Addictions Services as needed.
The goals of the Supported Housing is to help program participants obtain and remain in permanent housing, help participants increase skills and or income which will allow them to live as independently as possible. Also to help the participant achieve a greater sense of self determination to help gain the needed confidence to transition out of homelessness.
The Supported Housing is currently capable of serving twenty individuals who are assisted in finding and obtaining one to three bedroom apartments in Buffalo and other nearby communities. Other services provided include rental subsidies , security deposits, furnishings and appliances. Also, assistance is provided to help ensure residents are receiving services that they are entitled to (i.e. veteran and social security benefits). The Supportive Housing Program staff meet with residents on a regularly scheduled basis to ensure quality of care is being provided.
