Caz Recovery is partnering with the Western New York Integrated Care Collaborative (WNYICC) to provide Social Care Network services.
Screening and Navigation
- Screening and navigation includes:
- Screening applicants to determine if they can receive services through the Social Care Network. If approved, applicants may receive Level 1 or Level 2 services
- Level 1 services may include navigation to existing federal, state, or local resources (e.g., SNAP, current housing services, and others)
- Level 2 services may involve enhanced Health Related Social Needs services provided within the Social Care Network with a Social Care Plan
- Level 1 and Level 2 navigation both involve relevant referrals, assistance with applications, and follow-up
